Routledge Studies in Health in Africa

Over the past 30 years, an ever-expanding community of global health actors have been intervening in the poorest parts of the world with the aim of combatting disease and improving health outcomes. While many global health programmes in the Global South have been successful in saving lives, their successes have often been compromised by poor coordination, inefficiency and a lack of alignment with national priorities.This book outlines the deep historical and political dimensions of this fragmentation, using rural Zambia as a case study. In the name of global health, different actors produce and circulate radically different stories about how to improve health. These narratives not only contribute to a fragmented landscape of health provision but also create more subtle and pervasive forms of fragmentation in the everyday lives of health workers, patients and government officials. Based on long-term ethnographic fieldwork in rural Zambia, this book examines three different global health interventions driven by differing stories of salvation, community and experimentation. Tracing the longer history of these stories, the book shows how they have failed to account for the realities of life in rural Zambia.Providing an innovative and nuanced analysis of global health that challenges the idea of fragmentation as simply a technical problem, this book will be an important read for researchers of global health and African studies.

Originally published in 1974, this book filled a gap in its examination and evaluation of the way in which English local government forms a part of democratic society. Modern society is complex and although local government brings many benefits to ordinary people, it also circumscribes what they may do, particularly with regard to planning, education, housing, conservation and the environment. Local government is judged by its ability to raise the necessary finance, set its priorities and successfully provide services for local people. It is also judged by those democratic criteria of justice, impartiality, fairness and openness that are a mark of democratic society in all its aspects. This book examines these issues in the light of the reformed local government of the late 20th Century and traces the roots of the present system into the ideals and achievements of the great Victorian reformers.

Why do we use eighty-year-old metrics to understand today’s economy? The ways that statisticians and governments measure the economy were developed in the 1940s, when the urgent economic problems were entirely different from those of today. In The Measure of Progress, Diane Coyle argues that the framework underpinning today’s economic statistics is so outdated that it functions as a distorting lens, or even a set of blinkers. When policymakers rely on such an antiquated conceptual tool, how can they measure, understand, and respond with any precision to what is happening in today’s digital economy? Coyle makes the case for a new framework, one that takes into consideration current economic realities. Coyle explains why economic statistics matter. They are essential for guiding better economic policies; they involve questions of freedom, justice, life, and death. Governments use statistics that affect people’s lives in ways large and small. The metrics for economic growth were developed when a lack of physical rather than natural capital was the binding constraint on growth, intangible value was less important, and the pressing economic policy challenge was managing demand rather than supply. Today’s challenges are different. Growth in living standards in rich economies has slowed, despite remarkable innovation, particularly in digital technologies. As a result, politics is contentious and democracy strained. Coyle argues that to understand the current economy, we need different data collected in a different framework of categories and definitions, and she offers some suggestions about what this would entail. Only with a new approach to measurement will we be able to achieve the right kind of growth for the benefit of all.
