In this book, readers will learn learn about the equity and ethical&nbsp;issues associated with the practice of medical tourism in the Caribbean. Never before has a book discussed medical tourism with an exclusive focus on the opportunities&nbsp;and challenges it poses in the Caribbean region. As a region heavily dependent on tourism for economic sustainability, many Caribbean nations are looking to diversify their tourism sectors through involvement in medical tourism. In nine chapters, the book examines medical tourism through the lens of the Caribbean region to understand how and why this offshoring sector is unfolding in the way is and to consider who benefits from and is harmed by this global health services practice.&nbsp; This book is based on the author's first-hand research throughout the Caribbean region. The chapters are framed around specific insights she has gleaned&nbsp;about how medical tourism is unfolding in the region and its ethical&nbsp;and&nbsp;implications. Chapter 1 gives an overview of the global health services practice of medical tourism, and briefly introduces readers to the Caribbean region. Chapter 2 offers criticisms about the reliability of the information made available about medical tourism, not just in the Caribbean but also globally. Chapters 3 through 7 examine the complexities associated with competition&nbsp;between countries, the movement of locals and tourists between islands, the intersection of medical tourism and mobilities such as offshore medical schools, and the networks of impact resulting from the rise of medical tourism. Chapters 8 and 9 introduce the idea that there are 'winners' and 'losers' associated with medical tourism throughout the Caribbean region and that there is complexity with determining who falls into which group. This book will be of interest to students, academics, researchers, and policy makers involved with medical geography, global health, and health policy.&nbsp;

This book reflects new developments in the modeling and control of piezoelectric cantilevers. The main nonlinearities (hysteresis and creep) of these actuators are explained from a physical point of view and introduced into the modeling through operators and systems. The achieved models are adapted and used for the synthesis of classical (PID…) and advanced control schemes and laws. Another important aim of the control part in this book is the feed forward control which is of great interest for linearization and for damping vibration or for performances improvement when no sensor is available for feedback. Finally, the last part addresses the measurement techniques and integration for piezoelectric actuators. The concept of self-sensing and the use of observer techniques are highlighted, emphasizing the close relation between the nonlinearities and the reliability of the provided measurement. This book supplies practical tools for modeling, control and signal measurement of piezoelectric cantilevers. Numercial examples, identification and implementation procedures for experiments are provided. Finally, the appendices provide the tools to get familiar with the employed control techniques.

Governments around the world are privatizing everything, with dramatic but largely ignored negative consequences for human rights. This open access book outlines the response that is urgently needed. Corporations are being given control over water, healthcare, housing, public transportation, child welfare services, aged care, and much else. Privatization is promoted by international financial institutions, consulting firms, and development actors as the answer to public finances devastated by the very tax cuts insistently pushed by these groups. Their pitch is based upon a mythology that extols the virtues of an idealized market while ignoring the heavy human rights costs incurred. Building on the latest evidence and on specially commissioned case studies, this book shows how the touted 'efficiency' of the private sector is often predicated on higher charges and/or reduced services, destructive employment practices, and the highly predictable exclusion of many users. Privatization generally costs states more money, marginalizes democratic decision-making, and transforms citizens from rights-holders into customers while sidelining accountability. The authors develop an approach designed to enable human rights actors, including courts, UN treaty monitoring bodies, and NGOs to move beyond their longstanding agnosticism towards privatization. It starts with a presumption that privatization is inherently retrogressive in human rights terms. Whatever mix of public and private is reflected in any given arrangement, governments must retain the degree of control necessary to ensure respect for rights, and the legal, financial and administrative ability to do so. The book charts a new direction for responding to the ever-growing threat that privatization poses to human rights. This is the first book to present a new approach of institutional design as an evolutionary applied policy based on coherent conceptualization and methodology, and to study the actuality and implications of diversifying real/virtual community currencies. In contrast to a de facto standard theory, an institutional ecosystem can be composed of various institutions that continue to coexist with changing relative frequencies. In this view, diversified monetary institutions with local rules are feasible and realistic. Community currencies are worldwide attempts to redress such problems as financial instability, chronic stagnation, and decline of communities caused by globalization of the economy. Considering those alternative movements as a new type of policy, "evolutionist institutional design", this book clarifies the feasibility and significance of community currencies in the current era of globalization. The book also provides a better understanding than mainstream approaches of such platform institutions as money that determine fundamental means of cognition and action of interacting agents. Money is a symbolically generalized communication medium in the mesodomain that mediates between socioeconomic performance and patterns in the macrodomain and motives and cognitive frameworks in the microdomain. Conventional national systems of money and finance are platform institutions that determine basic replicators for a capitalistic market economy to evolve. In contrast, community currencies conveying both economic and social/cultural values are distinct platform institutions that are expected to endogenously change such basic replicators in outer and inner institutions for agents to a noncapitalistic market economy. These currencies are considered target institutions for "media design" in the framework of evolutionist institutional design.

This book focuses on a very important and still controversial topic in Japan, namely, whether a future exists for Japanese farmers, especially rice-producing farmers. Dealt with here is the prospect along with a personal view of the course that Japanese agriculture should follow, that is, a prescription for enriching the international competitiveness of the agricultural sector, especially rice production, in a more market-oriented economy. Toward restoration of Japanese agriculture, prescriptions are pursued from different aspects. One is the planting schedule of various farm products at the micro level such as the crop rotation system, derived from the mixed-integer dynamic programming model. Another is a macroeconomic policy to enhance the export of the restored 2 million tons of rice by the abolition of the policy of reducing rice acreage-a policy considered unreasonable by some. The path that agriculture in Japan should take has been argued by scholars and experts in related fields. However, no argument has been based on scientific and objective proof. Some applications of mixed-integer dynamic programming model are shown, which are very useful for policy makers as well as graduate students in the field of economic policy (esp. regional &amp; dynamics) and dynamic agricultural planning (esp. planting &amp; comprehensive management).
